(Fax Instructions)

NOTE: This option only available to US and Canada clients.

To place a Checks by Fax order, you will need to follow these directions:

1. Please click here to open the Fax Form and print the form by clicking on "File" in the upper toolbar of your browser and scrolling down to the "Print" command and click. Follow computer directions for printing.

2. Please fill out the Fax Form and attach a signed check marked "VOID".

3. You will next need to print out one of the following order forms and complete it:

   1. Colloidal Silver Generator Order Form (7kb)

   2. Small Blank Order Form (6kb)

   3. Complete Pre-printed Order Form (90kb)

4. Finally, fax both the Order Form and the Fax Form to
   1-877-FAX-8080 or 1-623-321-9977 for international. That's it! Your order will processed and filled in a timely manner.
